Location Preference: Do you prefer our Downtown Homewood clinic or our Chicago South Side location?
Currently, we only accept PPO insurance.
Early Intervention (E.I.): If your child is under age 3, please let us know if they are already enrolled in the Illinois E.I. program.
Evaluation: If you have any recent evaluations or an IEP from your child’s school, having those ready will help us tailor our first session.
